Pediatric Clinical Care Navigation Social Worker - Toledo Hospital - Full Time - Days
Are you a compassionate, driven professional with a passion for helping people through life’s toughest challenges? We’re looking for a Clinical Social Worker to provide critical psychosocial support to high-risk patients and their families dealing with complex emotional, mental, and physical health needs—including trauma, mental health crises, substance use, protective services, and grief. In this vital role, you'll serve as a trusted advocate and care partner, conducting clinical assessments, creating thoughtful care plans, delivering therapeutic interventions, and ensuring smooth transitions across the continuum of care. Your work will directly support patient safety, emotional wellbeing, and long-term healing. What You’ll Do Deliver advanced clinical social work services tailored to patients and families navigating serious stressors, illness, injury, or interpersonal conflict. Perform comprehensive psychosocial assessments and implement individualized care strategies. Collaborate closely with interdisciplinary healthcare teams to ensure coordinated, holistic care. Serve as a resource and advocate, connecting patients with essential hospital and community support systems. Balance a dynamic caseload in a fast-paced environment, providing crisis intervention, counseling, and transitional support. Education: Master’s Degree in Social Work (MSW) required. License: Current LSW (Licensed Social Worker) in the State of Ohio Skills: Strong clinical judgment and therapeutic intervention expertise. A history of building trust and rapport with patients, families, and care teams. Excellent problem-solving, crisis management, and communication skills. Tech-savvy, with comfort using Microsoft tools and electronic health records (EHRs). Deep knowledge of ethical standards, healthcare regulations, and local social service resources. Adaptability in a rapidly changing healthcare environment. Preferred Qualifications Experience: 1–2 years in a medical or hospital setting. Skills: Background in patient advocacy, care management, and navigation of hospital/community resources. License: LISW (Licensed Independent Social Worker) preferred.
