OverviewThe Field Supervisor is responsible to meet with the HCBS Participant and Attendant in their home to evaluate and assess the needs of the Participant. The Field Supervisor is to schedule timely follow-up visits to ensure the delivery of tasks assigned to the Participant care plan. They will complete all necessary paperwork to meet regulatory and company requirements and communicate pertinent information to the Branch Manager and administrative staff.
External Job Description Complete supervisory visits per company/contract specificationsComplete all necessary documentation for supervisory visits to meet regulatory and company requirementsAssist the Participant with any issues or questions relayed during the home visitReport issues, concerns, and problems to Branch Manager and administrative staff.Enter home visit information in the correct systemsDocument and track all home visit contact information with the ParticipantSupport Administrative staff when requested by SupervisorCooperate in external/internal investigationsSupervise and evaluate staffMonitor staff, identify problem situations, and develop/implement solutions for optimum outcomesEvaluate job performance of Attendants and conduct performance reviewsResponsibilities may include assigning tasks and providing supervision to other Branch or Administrative EmployeesQualifications Excellent customer service and communication skills; must be able to function well under pressure, make decisions quickly, and manage a large amount of informationDirect care and supervisory experience working with vulnerable populationsExcellent customer service, communication, and problem-solving skillsFunction well under pressure, make decisions quickly, and manage confidential informationSolid experience working with the public, presenting a friendly, professional appearance while interacting with peopleAbility to communicate both verbal and writtenCapable of working responsibly with confidential informationAbility to work independentlyAbility to work in a home-like settingSkilled at assisting others and accepting directionAvailable after office hours on-call, and weekendsAbility to service participants when attendant is not available About our Line of Business

Additional Job InformationREQUIRED:
Have at least two (2) years of supervisory experience in programs dealing with elderly and disabled individualsAND Satisfy at least one of the following:(1) Have a bachelor’s degree in social work, home economics, or a related profession with two (2) years of direct experience working with aged and disabled persons, or2) A licensed RN or Licensed Practical Nurse (LPN) with two (2) years of direct experience working with aged and disabled persons, or(3) Be a high school graduate with diploma or have a General Educational Development (GED) certificate, and four (4) years of direct experience working with aged and disabled persons.
