pertaining to questionsconcerns on all provider orders referrals Provides support to clinical staff in order to facilitate the administrative components of clinical referrals for various services Acts as a liaison between hospital physicians health plans vendors and patients or other referral services Manages all outgoing referrals to appropriate agencies to assist the client in achieving the goals and objectives identified in their Care PlanJob ResponsibilitiesGathers pertinent information from insurance carriers financial counselors and other ancillary staff to make certain the patientrsquos financial obligations for services providedProcesses paperwork for new patients efficiently and in accordance with protocols Verifies insurance coverage and obtains authorizations if needed from insurance carrier Resolves pre certification registration and case related concerns prior to a patientrsquos appointmentTracks and support patients when they obtain services outside the practice eg Emergency departments Hospital and other health care facilitiesMonitors and meets PCMH referral processesEnters referrals and documents communications actions and other data in an information system Ensures that all referral requests are properly signed and filedCare coordination ndash assists with specialty appointments and obtain needed tests recommended by specialistCommunicates with other team members and referral teams constructively in order to reduce conflicts and enhance the resolution of issuesBe the system navigator and point of contact for patients and families with patients and families having direct access for asking questions and raising concerns on orders and referralsWorking collaboratively with other team members provide care management services for high risk patientsObtains insurance referrals for patients getting PA for diagnostic imaging and sending referrals to specialist office for cardiac and preventative screeningsAssist and promote preventive care
