What Patient Navigator contributes to Cardinal Health
Practice Operations Management oversees the business and administrative operations of a medical practice.
Patient Navigators focus exclusively on the needs of patients and help resolve concerns about the quality of care so they can feel all their needs are being met. This position will work with multiple departments inside and outside the organization to address the patient’s needs and concerns.
Responsibilities
Performs assessment of patient care needs for new and ongoing patients.
Work with physician and clinical office to facilitate, coordinate and ensure patient has access to available resources.
Serve as an essential link between patients and all other providers. Central point of contact for all treatment patients; including interaction with medical, nursing, ancillary services.
Collect and report data from patient on barriers and intervene as appropriate for optimal patient care.
Assist patients with referrals to outside resources and care coordination as needed.
Follows established departmental policies, procedures and continuous quality improvement objectives.
Collaborates with medical providers, patient care staff and clinic management in the planning and implementation of patient and staff education.
Designs or participates in the creation of educational materials.
Performs miscellaneous job-related duties as assigned.
Qualifications
0-2 years of experience, preferred
Bachelor's degree in related field, or equivalent work experience, preferred
Candidates with direct patient care or Social Work experience, highly desired
Ability to calmly and professionally resolve patient issues with diplomacy and tact
Bilingual in Spanish, highly desired

Works on projects of limited scope and complexity
Follows established procedures to resolve readily identifiable technical problems
Works under direct supervision and receives detailed instructions
Develops competence by performing structured work assignments
Anticipated hourly range: $24.50 per hour - $36.64per hour
