Job Summary and Responsibilities

As our Patient Logistics Nurse, you will orchestrate seamless patient movement, optimizing bed utilization, coordinating transfers, and ensuring efficient patient flow. Your role is pivotal in minimizing delays and enhancing operational efficiency.

Every day you will manage and prioritize patient admissions, discharges, and transfers based on medical necessity and bed availability. You will proactively communicate with clinical teams, discharge planners, and external facilities to resolve logistical challenges and ensure a smooth continuum of care.

To be successful in this role, you need strong clinical judgment, exceptional organizational skills, and the ability to make rapid, informed decisions in a dynamic environment.

Receives bed placement requests, evaluates all available hospital beds and initiates, promotes and/or participates in interdisciplinary collaboration regarding patient placement; keeps abreast of bed demand/availability/status information in order to ensure patients are placed in the appropriate bed at the right time.Develops contingency patient placement plans with operational charge staff to ensure daily patient operations are not interrupted; continually re-evaluates requests as necessary to assure timely and appropriate bed assignments and optimal turnaround.Monitors staffing and patient information to maintain real-time awareness patient demands, including patient location, acuity, risk factors, and turnaround times.Maintains communication with appropriate stakeholders regarding bed availability or access delays; facilitates ongoing communication across work units regarding patient flow/bed placement status/issues.Identifies anticipated patient admissions/transfers/discharges and ascertains potential bed availability on an organizational basis; maintains ongoing communication with the Emergency Departments, other requesting units and Transfer Center to identify shifting priorities/needs for beds.Gathers clinical information to clarify clinical acuity and other relevant information bearing on bed and/or hospital placement that is consistent with criteria for the patient’s needs.

Nights:  7pm-7am, e/o weekend
