The patient flow registered nurse is an exemplar for clinical excellence, professionalism, compassion, acceptance, respect and empathy. Clinical proficiency is based on a sound knowledge base together with the understanding of the theories and science of nursing practice. Patient flow registered nurse job duties incorporate knowledge, skills and critical thinking abilities that support and guide efficient patient throughput. They must be a skilled communicator with the ability to work collaboratively with patients, families, providers, nursing departments, case managers, house supervisors, leaders and other stakeholders to positively affect patient throughput from admission to discharge. The patient flow registered nurse reflects expertise in working through all aspects of the throughput process including anticipating and addressing barriers as well as providing input into refinement of processes that enhance patient throughput. This role serves as a mentor to colleagues as a throughput expert.
Assessment and identification of patient needs and implementation of appropriate interventions considering the needs of patient populations per department scope of service.Delegation of nursing care activities and provisions of supervision; establishes and maintains a plan for the delivery of nursing care; assures that nursing care interventions and activities are directed to resolve problems; and performs technical aspects of care competently.Identifies and addresses educational needs of patients and families.Performs effective hand off communication to support continuity of care.Participates in discharge planning through collaboration with case management and other members of the healthcare team to promote a timely discharge.Completion of chart audits to ensure plans of care are updated especially as related to admission and discharge information and data.Attends Multidisciplinary Rounds (MDR) daily huddles, representing departmental nursing.Facilitates patient throughput with a special focus on admissions and discharges.Assists with readmission prevention activities and education.Serves as a mentor and role model to other staff members and nursing students.Supports and participates in the orientation of new staffServes as expert to primary nurses in best practices related to patient discharge activities and processesRequirements:
Bachelor’s Degree in Nursing or Associate Degree in Nursing with enrollment into an accredited Bachelor’s Degree nursing program within one year of hire and completion of program within 3 years of hire required.Current RN licensure recognized by the Kentucky State Board of Nursing is required.Current RN licensure or temporary Kentucky work permitCompletion and maintenance of BLS.Successful dysrhythmia testing within 3 months of hireACLS within 3 months of hire.NIHSS within 3 months of hire.Successful completion of all competencies as identified on the departmental nurse competency grid.Benefits:
