Physical Location/Address of Clinic: Riley Maternity Tower and Riley Fetal Center Hours: Mon-Fri 8:00am-4:30pm No weekends: required to work one Saturday each September No holidays Fluent in English and Spanish would be preferred We are a small team and the candidate would need to be a collaborative and flexible. We move between units to cover for each other. The units we work on are large and have hundreds of team members. We would need a candidate who is self-motivated and can create positive work relationships. Our entire team is comprised of parents/caretakers of children with chronic health care needs. This would be experience we prefer.
Neonatal Intensive Care Unit
Provide consultation and application assistance as an Indiana Navigator and Federally Certified Application Counselor; maintains current requirements. Assist patients and families with understanding their current health care financing options and the identification of other resources as needed. Remain current on regulations, programs, resources, benefits, services and policies related to health care financing for children and their families. Serves as a representative for NICU Family Support Program in professional and community organizations. Offer support, education and resources to families whose babies are in the Riley Maternity Tower NICU Facilitate communication among families and the health care team Provide parent-to-parent welcome and overview of the unit Provide families with parent-to-parent support resources and/or other services for children with ongoing health care needs Follow up with families after discharge from the unit Plan and implement activities, special events, and parent education for NICU families Collaborate with other family support staff to present annual NICU Fun Walk Consult with and share resources for health care financing for parents and other children Maintaining accurate charting of patient and family-specific services High-Risk Maternal-Fetal Medicine Program Offer support, education and resources to antepartum mothers on bedrest in the hospital Offer support, education and resources to families with a high-risk pregnancy and/or prenatal diagnosis upon referral Provide information on Medicaid and other health care financing options to high-risk and antepartum patients as needed Facilitate communication among families and the health care team Managing scheduling and provision of orientation/tours to labor/delivery, NICU and Riley Hospital for Children Provide support to high-risk mothers once admitted for delivery; provide information and communication with Riley NICU Represent NICU family support program at IU OB/GYN meetings Maintaining accurate charting of patient and family-specific services Facilitate bedrest support group and special events
Cares for patients as part of a care management program. Responsible for patient caseloads through in-person, telephonic, and electronic communications. Conducts outreach calls with patients and caregivers to encourage program participation. Assists patients with self-care needs including addressing language and cultural barriers as well as coaching patients to meet personal and clinical goals. Identifies patients in need of intensive case management. Supports patients in scheduling provider visits, arranging transportation, and creating office visit agendas (what to ask, what to expect, etc). Educates patients about preventative monitoring and caregivers about system response plans. Helps patient access community and governmental based service agencies including possibly filling out patient paperwork. Documents activities and reconciles patient information to support productivity of clinical care team. Collaborates with the multidisciplinary clinical care team. Administers program evaluation and conducts patient interviews. Evaluates program processes and provides recommendations for improvement.
High school diploma or equivalent required. Bachelor's degree preferred. MA, LPN, BSW, or Clinical Dietitian and appropriate accreditation/licensure preferred. Experience working with patients who have chronic and behavioral health needs. Demonstrated experience working as part of a multidisciplinary clinical team.
