General NotesThe Dell Medical School is seeking a Patient Care Navigator.
PurposeThe Patient Care Navigator supports patients in navigating the healthcare system, ensuring they receive timely access to medical services and resources. Reporting to the Nurse Manager, the Navigator collaborates closely with physicians, nurses, social workers, and administrative staff. Key responsibilities include coordinating appointments, providing health education, and advocating for patient needs.
ResponsibilitiesCare Coordination
Schedules and coordinates patient appointments and follow-ups.
Manages referrals to specialists and other healthcare providers.
Ensures seamless transitions between different stages of care.
Patient Advocacy
Advocates for patients' needs and preferences.
Provides emotional support and guidance throughout the healthcare journey.
Addresses barriers to care, including financial and logistical challenges.
Health Education
Educates patients about their medical conditions and treatment options.
Provides information on self-care strategies and preventive measures.
Conducts health education sessions for patients and families.
Resource Referral
Connects patients with community resources and support services.
Assists with accessing financial assistance programs.
Refers patients to social services and other relevant resources

Required QualificationsRequires a Bachelor's Degree in Healthcare Administration, Social Work, or a related field with at least 3 year(s) of experience in healthcare setting.
Relevant education and experience may be substituted as appropriate
Preferred QualificationsMaster's Degree in Healthcare Administration, Social Work, or a related field. with at least 2 year(s) of experience in patient navigation or care coordination
Salary Range$47,000+ depending on qualifications
