We are seeking a Patient Care Manager Specialist to join our team, reporting to the Regional Director of Operational Excellence. This critical role is responsible for managing and overseeing patient care activities across assigned hospice markets to ensure delivery of high-quality care while supporting compliance with company policies and regulatory requirements.

 

 

As a Patient Care Manager Specialist, You Will:

Provide oversight for patient care delivery across designated marketsManage rescheduled, declined, missed, and reassigned visit requests based on clinical prioritiesTriage phone inquiries and medical concerns within scope of practiceFacilitate Interdisciplinary Group (IDG) meetings, review patient conditions, and ensure accurate care planningCoach, evaluate, and support team members to ensure regulatory compliance and clinical best practicesPromote documentation accuracy, compliance, and timelinessHelp build a culture of continuous improvement, compassion, and accountabilitySupport adherence to local, state, and federal regulatory requirements (Medicare, Medicaid, ACHC, JCAHO, etc.) About You

Qualifications – What You’ll Bring:

Current RN license in state of residence and employmentAt least 3 years of direct patient care experienceMinimum 2 years of experience in a hospice or home-based care settingStrong understanding of hospice principles, best practices, and regulatory complianceAbility to lead interdisciplinary meetings and support diverse clinical teamsExcellent communication, problem-solving, and decision-making skillsDetail-oriented with strong documentation and organizational capabilities

 

Preferred Experience (Not required):

Prior experience in clinical leadership or hospice program managementFamiliarity with IDG processes and electronic medical record systemsExperience supporting multiple hospice sites or regionsAdvanced knowledge of CMS hospice regulations and audit preparednessSkilled at coaching staff and fostering professional development in clinical teams We Offer
