Job Description
The role of a Colon Screening Patient Care Coordinator is essential in ensuring seamless communication and coordination among clinic providers, patients, families, and various healthcare entities. The coordinator acts as a liaison to facilitate pre-operative planning, preparation, and hospitalization activities.
Responsibilities
+ Serve as the primary contact for clinic providers, patients, families, referring physicians, and outside agencies, providing information about the clinic's services.
+ Act as a liaison between patients and clinic areas to coordinate pre-operative planning and preparation.
+ Coordinate pre-admission tests, pre-surgery appointments, and procedures as delegated by providers or nurses.
+ Ensure the presence of a complete medical record at the time of patient visits, admissions, or procedures.
+ Facilitate the flow of documents between patients, providers, and payers.
+ Communicate with healthcare team members about the status and completion of care coordination activities.
+ Ensure timely addressing of internal referrals in specialty clinics under the direction of clinic leadership.
+ Collaborate with financial services to ensure financial information is complete before appointments, procedures, or admissions.
+ Assist with scheduling surgeries, procedures, or services needed for ambulatory management of patient care.
+ Promote and protect general safety for patients, families, friends, and co-workers through adherence to correct procedures.
+ Provide summary reports to clinic leadership on referral volumes and status where applicable.
+ Support clinic and divisional efficiency through participation in quality assessment and improvement activities.
+ Apply appropriate policies, procedures, protocols, and standards of practice in performing duties.
+ Promote the organization's mission externally and function as an ambassador when interfacing outside the organization.
+ Consistently demonstrate the organization's values, including patient-centered care and service, respect, caring, compassion, teamwork, partnership, continuous learning, improvement, and leadership.
+ Utilize the AIDET system for excellent service.
Essential Skills
+ Minimum of 1 year experience in a medical/healthcare setting or 2 years in a customer service role.
+ Experience providing a high level of customer service in a fast-paced healthcare environment, including handling confidential data with discretion.
+ Proficiency with Electronic Medical Record systems, including EMR, Epic, and care coordination.
+ Knowledge of medical terminology, CPT, ICD10 coding, insurance authorization, and prior authorization.
+ Solid understanding of medical terminology and CPT and ICD10 coding.
+ Ability to manage confidential information.

Pay and Benefits
The pay range for this position is $22.00 - $26.00/hr.
