Job Description
+ The Colon Screening Patient Care Coordinator plays a vital role in ensuring seamless communication and coordination among clinic providers, patients, families, and external healthcare entities. This position serves as a liaison to support pre-operative planning, preparation, and hospitalization activities.
Responsibilities
+ Serve as the primary contact for clinic providers, patients, families, referring physicians, and outside agencies
+ Act as a liaison between patients and clinic areas to coordinate pre-operative planning and preparation
+ Coordinate pre-admission tests, pre-surgery appointments, and procedures as delegated by providers or nurses
+ Ensure complete medical records are available for patient visits, admissions, or procedures
+ Facilitate document flow between patients, providers, and payers
+ Communicate with healthcare team members regarding care coordination status
+ Address internal referrals in specialty clinics under clinic leadership direction
+ Collaborate with financial services to confirm financial information before appointments or admissions
+ Assist with scheduling surgeries, procedures, or ambulatory services
+ Promote safety for patients, families, and staff by following proper procedures
+ Provide referral volume and status reports to clinic leadership
+ Support clinic efficiency through quality assessment and improvement activities
+ Apply relevant policies, procedures, and standards of practice
+ Represent the organization externally and act as an ambassador
+ Demonstrate core values: patient-centered care, respect, compassion, teamwork, learning, and leadership
+ Utilize the AIDET system to deliver excellent service
Essential Skills
+ Minimum 1 year in a medical/healthcare setting or 2 years in customer service
+ Strong customer service experience in a fast-paced healthcare environment
+ Proficiency with EMR systems, including Epic and care coordination tools
+ Knowledge of medical terminology, CPT, ICD10 coding, insurance authorization, and prior authorization
+ Ability to manage confidential information with discretion

Pay and Benefits
The pay range for this position is $22.00 - $26.00/hr.
