Patient and Family Navigator, Blue Ridge Care Connection for Children

The Department of Pediatrics at the University of Virginia’s School of Medicine seeks applicants for a Patient and Family Navigator to support the Blue Ridge Care Connection for Children. The Blue Ridge Care Connection for Children program provides community-based services and funds to support families with children with special healthcare needs. Our professional care coordinators help children, and their families access medical and community-based services targeted to each child's individual needs.

The ideal candidate will be responsible for working with patients and families to provide support and promote care coordination and system navigation between health care providers and community agencies and resources. The ideal candidate will also assist with problem solving of complex patient care issues. 

More than 190,000 children in Virginia have special healthcare needs — physical disorders that are expected to last for at least 12 months. These children need treatment beyond the norm and may face social, academic, or athletic challenges. Six regional programs operate throughout the Commonwealth. Each program is managed locally, maximizing the program's ability to make efficient and effective use of the area's unique resources. 

Job Responsibilities: 

Provide support and assistance to patients and families during the intake process (e.g., assistance with completing questionnaires and intake forms). Assists patients and families in being proactive regarding their health care. Assist in learning to communicate effectively with health care educators and other service providers. Assist patients and families in identifying and prioritizing their needs. Provide care coordination and resource navigation services using a patient- and family-centered approach. Identify and address service gaps and barriers that make it difficult for patients and families to meet their health care needs.Communicate with the clinical team regarding status of referrals, outreach, and care coordination efforts. Compile and maintain up-to-date lists of local and statewide resources as appropriate. Collect, update, and/or create resource materials to foster understanding of patient care needs. Develop and maintain ongoing connections with community resources, support groups and other service providers. Collaborate with community partners to share current resource information and advocacy approaches. Facilitate patient enrollment and participation in any research or other relevant health care initiatives as appropriate. Assist with planning and implementation of patient and family education and support activities, workshops, and trainings. Assists with problem solving of complex patient care issues. Investigates, documents and resolves or refers as appropriate problems and conflicts. Consistently evaluates processes, identifies opportunities for improvement and offers constructive ideas and solutions; actively participates in solutions and implementation. In addition to the above job responsibilities, other duties may be assigned. 

 
Minimum Qualifications: 

Education: Bachelor’s Degree or equivalent combination of experience, education, licensure/certifications may be accepted in lieu of degree.  Experience: One year of experience working with complex patient populations. 

 

Preferred Qualifications: 

Bachelor’s degree in Social Work, Psychology, Sociology or Public Health preferred. Two years progressive work experience, including experience working with complex patient populations, medical terminology, and customer service. 

 

Physical Demands 

Job requires frequent bending/stooping/walking. Proficient communicative, auditory, and visual skills; attention to detail and ability to write legibly; ability to lift/push/pull 20 - 50lbs. May be exposed to chemicals, blood/body fluids, and infectious disease. 

 

The anticipated hiring range is $49,000 - $58,000, commensurate with education and experience. 

 

Position Type & Work Location  

This is an exempt-level, benefited position. Learn more about UVA benefits.  This is a restricted position, which is dependent on funding and is contingent upon funding availability.  This position is based in Charlottesville, VA, and is eligible for a hybrid schedule, with additional on-site presence as needed, after required in-person training and in accordance with UVA’s remote work guidelines.
