Case Worker - Glen at Hiland Meadows

Part Time – 20 Hours- Days (some flexibility)

The Glen at Hiland Meadows is a beautiful senior living campus that provides a homelike, supportive environment for residents across Independent Living, Enhanced Assisted Living, and Memory Care.  Our multidisciplinary team is dedicated to enriching the lives of older adults and fostering a culture of dignity, respect and connection .

We are currently seeking a part time case worker to join our resident services team, working closely with our onsite Social Worker and other team members to ensure that residents and their families receive the support, advocacy, and resources they need.

Key responsibilities include:

-Advocate for residents’ emotional and social needs

-Assist with family/resident meetings

-Conduct initial and ongoing assessments of resident psychosocial needs

-Support through care level transitions

-Facilitate support groups and engagement activities

-Connect families with community resources

-Participate in trainings and multidisciplinary care meetings

Qualifications:

Bachelor’s degree in Social Work, sociology, psychology, LPN licensure or related field required.

Experience in social work, case management or related field preferred

Strong communication and organizational skills

Compassionate, team oriented, resident focused

Senior living experience a plus

Pay Range: $23.40 - $33.69

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
