**Palliative Care Coordinator - Administrative Assistant**
**JOB PURPOSE:**
To assist with administrative functions of the Palliative Care Coordination Program.
**KEY RESPONSIBILITIES:**
· Responsible for taking and processing Palliative Care referrals
· Provide support to the Palliative Care Nurse
· Will schedule visits to support goals of care as requested
· Coordinate and communicate patient care goals between all providers
· Determine eligibility for services, present the palliative program, obtain legal consents, and assess for interdisciplinary needs
· Complete all documentation accurately, timely, and in accordance with company policy and procedure
· Inform palliative team of all new admissions and discharges
**KNOWLEDGE, SKILLS, ABILITIES:**
· Possess excellent communication skills, both oral and written.
· Excellent organizational skills.
· Ability to handle confidential matters.
· Commitment to the Palliative/Hospice concept and standards
· Attend and participate in continuing educational programs to keep abreast of changes in your field as well as to maintain current license/certification, as required.
· Attend and participate in mandatory in-services.
· Honor patients/residents’ rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaints.
· Comply with corporate compliance program.
· Follow established safety regulations, to include fire protection & prevention, smoking regulations, infection control, etc.
· Follow established safety procedures when performing tasks and/or working with equipment.
Perform other related duties as necessary and as directed by supervisor.

**MINIMUM EDUCATION REQUIRED:**
Two yeas of college or business school and/or equivalent experience and training.
**MINIMUM EXPERIENCE REQUIRED:**
One year minimum experience.

4.
