Position Summary: Responsible for engaging individual youth & families within the organization and community for the purpose of enrollment. Conduct and coordinate outreach activities and programs that promote Youth Care Coordination services and create opportunities for enrollment and re-enrollment. Must have professional appearance, strong communication, organization, and interpersonal skills, timely documentation, and the ability to work independently. Must have working knowledge of computer systems (i.e., Outlook, Microsoft Office, Microsoft Excel, etc.).

Essential Accountabilities:

Demonstrate knowledge of childhood psychiatric disorders, chronic medical conditions, and complex trauma. Locate and engage, through various methods (including but not limited to: telephone, face to face visits, mailings, internet searches, etc.), individuals with chronic medical conditions, mental health issues, and/or substance use disorders, often co-occurring. Engage and participate in community events, tabling events, and perform telephone, in-person, Zoom, etc., community outreach to referral sources and prospective members. Conduct progressive outreach attempts for all referred clients at least weekly, providing telephonic as well as face to face outreach, engagement, and comprehensive service planning in the field. Abide by Skyward Health policy and procedures for Health Home Serving Children.  Collect eligibility documentation and initial appropriateness documentation needed to enroll a member.  Enroll a minimum of (approximately) 15+ clients into the health home serving children’s program per month; schedule and conduct intakes to complete all enrollment forms, as well as upload the forms to the EHR within 48 hours of member enrollment. As assigned, a caseload of 2 members (based on acuity) may be carried for a maximum of 2 months. In this case, the Outreach Worker will complete all initial workflow documentation. Responsible for completing all billing activities for members enrolled within the month of service.  Utilize Foothold, PSYCKES, MAPP, UAS, etc. for programmatic data entry and needs.  Maintain detailed, timely, and accurate record keeping in the medical record. Coordinate with Outreach Director, and youth care management team supervisors in a timely manner to ensure accurate caseload status (including enrollments and screen outs). Monitor Medicaid/MCO status for all outreach clients twice monthly; complete all required monthly documentation as required to ensure continuity of engaged clients’ medical insurance and to ensure appropriate and accurate billing. Participate in Youth Care Management and Outreach meetings to address agency policy updates, HHCM policy updates, paperwork, billing, trainings, resources, etc.  Be responsible for reporting/coordinating daily office and field schedules with other members of the team and supervisor. Attend periodic trainings to enhance skill level and to maintain DOH / HHSC training requirements Participate in bi-weekly individual supervision to address concerns/issues and improve skill development. Be responsible for agency vehicles, including upkeep, documentation, and gas card when assigned. Be responsible for agency cell phone, laptop, and associated items. Follow agency, DOH, OMIG, Health Home, etc. policies and procedures. Report to the program administration any issues and/or concerns on a regular/as needed basis while working in the field. Responsible for having access to an insured, personal vehicle. Track and submit mileage. Other duties as assigned.
