This position is responsible for supporting outreach and engagement activities, that include connecting with members to complete Health Risk Assessments and other needed services that support improved health outcomes. This position works in collaboration with Clinical Operations and Population Health to help support members with their health needs and social determinants of health. Supports the Centralized Outreach and Engagement model for the Center of Excellence.

Qualifications High School Diploma or GED required.1 year of healthcare experience requiredCall center experience preferredKnowledge of related medical terminologyBilingual-Spanish/English preferred Responsibilities Engage in both inbound and outbound communications with members across all lines of business, providing guidance and support related to care coordination, housing, food access, transportation, and other essential services.Identify and notify the care coordination team of member needs that require services from community-based agencies.Educate members on PHP and community-based programs to enhance their understanding of available healthcare services and resources.Conduct outreach to new Medicaid members, including the DSNP population, to facilitate the completion of HRAs and coordinate CNA appointments with Care Coordination.Assist members in navigating healthcare services and providing relevant resources to meet their specific healthcare needs.Encourage and motivate members to become active participants in managing their health and wellness.Accurately document all member interactions, services provided, and outcomes within internal systems to ensure proper tracking and reporting.Manage and maintain outreach-related information, ensuring data integrity and accessibility for leadership and reporting agencies.Process, respond to, and track intake forms and referrals while adhering to established regulatory requirements and organizational protocols.Assist in the development and implementation of outreach strategies to maximize member engagement.Conduct follow-up outreach to ensure members successfully connect with recommended services and programs.Perform additional duties and projects as assigned to support outreach and engagement initiatives.
