Outpatient Staff Pharmacist

This Outpatient Staff Pharmacist position is located at our onsite pharmacy in Ankeny, IA.

JOB SUMMARY / PURPOSE
Provide patient focused, outcome oriented pharmaceutical care to patients of all ages, including neonatal, infant, pediatric, adolescent, adult, and geriatric. Facilitate the timely distribution of drugs, and disseminate drug information to patients and health care professionals.

ESSENTIAL KEY JOB RESPONSIBILITIES
•    Deliver pharmaceutical care to patients.
•    Promote professionalism and continually strive to raise the level of professional practice.
•    Interpret physicians' orders as they relate to the patient's drug therapy.
•    Evaluate the appropriateness of medication orders based on clinical data available in the medical record (including computerized information) and provide recommendations to physicians when appropriate.
•    Monitor the patient's medication profile for allergies, contraindications, duplications of therapy, drug interactions, and potential adverse drug reactions.
•    Contact physicians and nurses to clarify orders or recommend changes in therapy as appropriate.
•    Participate in and deliver specialty pharmacy care management activities, as appropriate
•    Answer drug information questions.
•    Provide medication teaching to patients and families when requested or as indicated and with each new ambulatory prescription.
•    Monitor adherence to dosing guidelines and target drug protocols and intervene as necessary.
•    Maintain patients’ ambulatory profiles.
•    Participate in pharmacy quality management activities.
•    Conduct MTM interviews and document therapeutic and cost saving interventions.
•    Appropriately bill prescription claims to Third Party payers.
•    Oversee and assume responsibility of technician activities.
•    Oversee and interpret audits performed by technicians.
•    Assume responsibility for unit inspections in assigned areas.
•    Participate in experiential education of pharmacy students.
•    Enter medication orders onto the pharmacy computer system and complete other documents related to the dispensing and control of medications.
•    Prepare, compound, and dispense medications.
•    Take, compile and invoice orders for provider’s offices and/or EMS services. 
•    After certification, administer vaccines and document the administration in IRIS.
•    Check medication orders filled by pharmacy technicians.
•    Ensure adequate inventory of pharmaceuticals and other supplies.
•    Maintain adequate records of disbursements of controlled substances    .
•    Understand and comply with all state and federal pharmacy laws and regulations. Complete required hospital education annually.

MINIMUM QUALIFICATIONS
Required Education and Experience   

 •    Pharmacy degree (B.S. or Pharm.D.) from an ACPE approved college of pharmacy.
•    Successful completion of ASHP-accredited PGY-1 Pharmacy Practice Residency or 3 years of ambulatory pharmacy experience preferred.

Required Licensure and Certifications    

•    Licensed, or eligible for licensure, pharmacist in Iowa. Must be licensed within 3 months of hire. 
•    BLS certification within 3 months of hire. 
•    Documentation of successful education and qualification to order and administer vaccinations in the State of Iowa. 
•    Other certifications if requested by direct supervisors/managers. 
•    Proof of completion of Mandatory Reporter abuse training for child and dependent adult abuse within three (3) months of hire.

Required Minimum Knowledge, Skills, Abilities and Training Knowledge: Achieves and maintains an adequate knowledge base including pharmacology, therapeutics, pharmacokinetics, therapeutic drug monitoring, and literature evaluation to provide clinical services for all patient populations and age groups served. Provides clinical services as required by policy and procedure.

1.    Effective communication with patients and other healthcare professionals.
2.    A high level of analytic ability to research and evaluate drug information, interpret patient specific information, and resolve medication related problems. 
3.    Displays enthusiasm for teaching and is proficient in all preceptor roles (instructing, modeling, coaching, or facilitating). 
4.    Proficient in use of computers, software for pharmacy and healthcare applications, specific pharmacy practice technology and equipment.
5.    Efficiently and adequately document clinical activities and interventions such as consults, patient education, and other patient specific information as defined by policy and procedure.
6.    Ability to utilize problem solving skills to promote a positive work environment and interpersonal relationships, while maintaining professionalism and confidentiality.
