Outpatient Pharmacist

The Outpatient Pharmacist provides comprehensive pharmaceutical care within a high-touch ambulatory retail pharmacy, supporting medication access, prescription fulfillment, and transitions of care for patients across the continuum. Located on the second floor with convenient patient access, this role combines traditional outpatient pharmacy practice with enhanced clinical collaboration, allowing pharmacists to work at the top of their license. Responsibilities include verifying and dispensing prescriptions, performing clinical medication reviews and therapy assessments, counseling patients on safe and effective medication use, and resolving insurance, billing, and medication access issues. The pharmacist partners closely with physicians, nurses, care coordinators, and bedside teams to support discharge planning and continuity of care while supervising pharmacy technicians and ensuring efficient daily operations, regulatory compliance, and the highest standards of patient safety. **A caregiver in this position will work a compressed full-time schedule with rotating shifts of 7:30am-6:00pm and 8:30am-7:00pm, Monday through Friday. Caregivers receive a scheduled day off each Wednesday as part of the rotating schedule and participate in an every-fourth-Saturday rotation from 9:00am-1:00pm. The department is closed on Sundays and major holidays, with no on-call or routine float requirements.** A caregiver who excels in this role will: + Provide pharmacy services and ensure the appropriate selection, dosing, administration and dispensing of medication orders. + Provide safe, efficient, patient-centered, evidence-based medication services. + Verify medication orders and/or dispense medications accurately according to state and federal regulations. + Contribute to medication safety initiatives. + Document services in a complete, accurate and timely manner. + Ensure all pharmacy operations comply with national accreditation standards. + Maintain professional interactions with patients to facilitate successful patient experiences. + Educate and train students, residents, other caregivers, and/or patients. + Be accountable for drug use policy adherence. Minimum qualifications for the ideal future caregiver include: + PharmD or Bachelor's of Science in Pharmacy from an accredited College of Pharmacy + Current state licensure as a Registered Pharmacist (RPh) or eligible in current state within 90 days of hire + Basic Life Support (BLS) Certification through the American Heart Association (AHA) and completion of a Board of Pharmacy approved course in the administration of immunizations within 90 days of hire for those administering immunizations Preferred qualifications for the ideal future caregiver include: + Completion of a Post-Graduate Year 1 Residency (PGY1), fellowship or relevant post-graduate pharmacy experience + Post-graduate year 2 residency (PGY2) or relevant experience preferred for Clinical Pharmacists hired in a Specialized Pharmacy practice area + Board Certification + Licensed Pharmacist with ambulatory/outpatient pharmacy experience **Physical Requirements** : + Typical physical demands include ability to perform the job while standing and walking for prolonged times, approximately 75% of shift and continuously for extended periods of time. + Manual dexterity sufficient to perform computer and dispensing functions of the job; ability to lift up to 20 lbs. + Position often includes reaching, stretching, stooping, climbing steps and sitting. + Must have normal or corrected vision. + Must have ability to clearly communicate verbally by phone or in person **Personal Protective Equipment** : + Follows standard precautions using personal protective equipment as required. **Pay Range** Minimum Annual Salary: $101,900.00 Maximum Annual Salary: $155,390.00 The pay range displayed on this job posting reflects the anticipated range for new hires. A successful candidate's actual compensation will be determined after taking factors into consideration such as the candidate's work history, experience, skill set and education. The pay range displayed does not include any applicable pay practices (e.g., shift differentials, overtime, etc.).
