**Description:**
Nazareth Hospital, a member of Trinity Health Mid-Atlantic, is looking for an Occupational Therapist to join our team.
Performs evaluations and implements treatment in all levels of care (acute, TCU, acute rehab, outpatient). Delivers care in accordance with patient practice guidelines upon physician referral and under the supervision of the Senior Therapist. Adheres to all hospital and department policies including but not limited to: infection control, performance improvement, billing procedures and scheduling. Assists in the supervision of COTAs, students, aides and volunteers.
**Key Responsibilities:**
+ Perform comprehensive patient evaluations and develop individualized treatment plans with measurable goals.
+ Implement therapeutic interventions including ADL training, functional mobility, splinting, balance activities, and more.
+ Monitor patient progress and adjust treatment plans accordingly.
+ Collaborate with interdisciplinary teams for discharge planning and care coordination.
+ Educate patients and caregivers on therapy goals, safety precautions, and home programs.
+ Ensure patient safety and adhere to infection control protocols.
+ Supervise COTAs, students, aides, and volunteers as needed.
+ Participate in performance improvement initiatives and maintain accurate documentation.
**Qualifications:**
+ Bachelor’s degree in Occupational Therapy (Master’s or Doctorate preferred).
+ Current Occupational Therapy license in the state of Pennsylvania.
+ BCLS certification required.
+ 2 years of experience preferred.
