Specialist will collaborate as an integral member of the multidisciplinary team in the provision of care for patients in our medical clinics to optimize and promote rational safe evidence based and cost effective therapy In addition to direct patient care responsibilities the oncology pharmacy specialist will actively participate in committees and or workgroups collaborate in the review of new and existing medications clinical practice guidelines and other related policies used in the management of the clinic specific medical conditions This person also conducts clinical research and practice advancement projects as well as patient care quality and regulatory compliance initiatives designed to improve medication use processes or pharmacy practiceHartford HealthCarersquos Pharmacy PreferredLicensure Certification RegistrationCurrent unrestricted pharmacist license from Connecticut Board of Pharmacy or eligibility within daysBoard Certification within specialty practice within
