Position Summary:
The Nurse Navigator is a key member of the multidisciplinary team functioning as an advocate, educator and facilitator of care. Responsible for ensuring all referred patients receive quality and comprehensive services throughout the continuum. Coordinates the overall subspecialty programmatic goals and outcomes.

Bachelor’s degree in NursingPreferred: Master DegreeCurrent State of Michigan Registered Nursing LicenseBLS active certificationThree years of experience in an acute care setting.One year of experience in medical, surgical or radiation oncologyPreferred: Case management experience or patient care leadership experienceAbility to lift up to a maximum of 35lbs. and perform push/pull tasks that require a maximum force of 35 lbs. Ability to physically operate safe patient movement equipment in compliance with the Safe Patient Movement Policy.What you will do:
Completes a comprehensive health assessment of the patient’s physical, psychological, social, environmental, financial and functional status in collaboration with multidisciplinary team members. Includes assessment of past and present treatment courseAssists in organizing and coordinating timely patient appointmentsUnderstands and develops goals, treatment plan and provider options in concordance with evidence based standards and clinical trialsWorks directly with physician lead to review diagnostic studies, clinical history and course of treatment to identify most appropriate next course of careCollaborates with multidisciplinary team members in performance improvement activities including assessment, planning, implementation and evaluation of clinical outcomes and processes for patientsWhat you will work:
