Outpatient Home Care Skilled Nursing or Assisted Living environmentUpon Physician referral performs thorough evaluation of patientrsquos level of functionPerforms evaluations for upper extremity strength motor control joint mobility soft tissue integrity pain balance coordination perceptual cognitive motor function and functional abilities of daily living Recognizes physiological psychological and age related changes in the patient Consults with other Health Care Team members as necessaryDevelop an individualized patient centered treatment plan of careEvaluate care plan on an on going basis revising as necessary in accordance with patientrsquos care needs Establish measurable goals with the patientfamily based on appropriate patient care protocols and critical paths in order to meet patient needsNotify Physician of any changes in status or progressDeliver therapeutic techniques that are appropriate to patientrsquos needs and medical statusDeliver therapeutic techniques that are appropriate to patientrsquos needs and medical statusImplement deliver and delegate patient care by utilizing appropriate interventions and clinical decision making Therapeutic
