of contexts and environments in order to maximize function in the areas of daily living leisurerecreation and productivityworkDelegates patient care responsibilities to Certified Occupational Therapy AssistantThis role encompasses evaluation skills and techniques analysis of patients and application Occupational Therapy to maximize the patients functional independenceMay work in an Inpatient Outpatient Home Care Skilled Nursing or Assisted Living environmentUpon Physician referral performs thorough evaluation of patientrsquos level of functionPerforms evaluations for upper extremity strength motor control joint mobility soft tissue integrity pain balance coordination perceptual cognitive motor function and functional abilities of daily living Recognizes physiological psychological and age related changes in the patient Consults with other Health Care Team members as necessaryDevelop an individualized patient centered treatment plan of careEvaluate care plan on an on going basis revising as necessary in accordance with patientrsquos care needs Establish measurable goals with the patientfamily based on appropriate patient care protocols and critical paths in order to meet patient needsNotify Physician of any changes in status or progressDeliver therapeutic techniques that are appropriate to patientrsquos needs and medical statusDeliver therapeutic techniques that are appropriate to patientrsquos needs and medical statusImplement deliver and delegate patient care by utilizing appropriate interventions and clinical decision making Therapeutic techniques to support every day activities include but are not limited to fine motor skills exercises to improve gross motor skills and techniques to support and to maximize function in the areas of daily living leisurerecreation and productivityworkProvides patient and caregiver education regarding treatment precautions and equipment useCompletes all documentation including but not limited to evaluation care plan and goals patient status and progress visits patient discharge summary and patient care transition plan and summaryRequires the timelyand accurate documentation and coding of skilled and medically necessary services provide to the patientParticipate in patient transition planning andor patient discharge planning including recommendations to community resources to meet patient needs throughout the continuum of care Help to coordinate patient care with other care providers such as Rehab Therapists Nurses Social Workers and other community resources as related to care transition planningDelegate patient care tasks and provide clinical supervision for Occupational Therapist students Rehabilitation Aides and Certified Occupational Therapy AssistantsMaintain a clean neat work environment for patients and staff including adhering to infection control and safety precautions Restock supplies as necessaryMaintains clinical proficiency continuing education units CEUs
