The Occupational Therapist – Home Health provides in-home patient care across the west side of the Cleveland area (including Lakewood and Tremont), supporting a diverse population with orthopedic, neurological, and cardiovascular conditions. This role focuses on evaluations, start-of-care visits, and ongoing treatment, including discharge planning, wheelchair assessments, and durable medical equipment recommendations. Caregivers educate patients on safely completing daily activities to remain independent at home while managing their own caseload and following patients through the plan of care. Schedules are coordinated the day prior, allowing therapists to plan efficient routes, with documentation typically completed from home. This role also includes participation in interdisciplinary care conferences and team coordination.

A caregiver in this position will work 8:00am to 5:00pm with Saturday rotations every 5th weekend. 1 holiday and 1 holiday weekend are also on rotation.

A caregiver who excels in this role will:

Perform initial and ongoing assessments of patient’s condition.Establish, revise, and evaluate a plan of care which is appropriate to problems identified and involves the patient/family according to their capabilities and desires.Perform therapy interventions utilizing standard occupational therapy techniques and skills as appropriate for the age and condition of the patient.Supervise activities of occupational therapist assistants, students, and other support personnel.Always maintains complete and up-to-date documentation and files regarding patient condition and effectiveness and timeliness of care and interventions.Participate in ongoing interdisciplinary assessment of assigned patients. Participates as a member of the hospice interdisciplinary team when necessary.

Minimum qualifications for the ideal future caregiver include:

Bachelor's DegreeMust be a graduate of an Occupational Therapy Program accredited by the American Occupational Therapy AssociationCurrent State Licensure as an Occupational Therapist (OT)Current Basic Life Support (BLS) through American Heart Association (AHA) or American Red CrossValid Ohio Driver’s license upon hire; per policy 8-175 thereafterProof of automobile insurance coverage with $100,000/$300,000 coverage, upon hire; per policy 8-175 thereafter

Preferred qualifications for the ideal future caregiver include:

Experience in clinical areaTwo years of experience in an OT positionHome care experience

Physical Requirements:

Ability to walk without assistive device and to maintain balance despite sudden load in order to support patient from falling.Ability to meet physical demands characterized at the heavy level defined by 1993 Leonard Matheson and Ministry of Labor.Ability to lift and reach with arms sufficiently to lift patient's arm and leg during therapeutic treatment program.Sensation of touch and manual dexterity to be able to palpate muscle tension or to monitor patient pulse.Visual acuity to observe patient's gross and fine motor movements in order to analyze these movements and to read all patient monitors.Ability to speak clearly enough to instruct patients and families.Ability to hear at a conversational level to understand patient's description of symptoms and response to treatment.
