Manage time and resources effectively across a home-based caseloadAdhere to organizational policies and productivity expectationsSupport efficient care delivery and optimal patient outcomesEducation & ExperienceMaster’s Degree in Occupational TherapyGraduate of an AOTA-accredited programMinimum 1 year of clinical experience (or strong clinical internship background)Current Illinois OT license (or eligibility)Licensure & CertificationsLicensed Occupational Therapist – State of IllinoisBLS certification (or obtained within 6 months)Valid driver’s licenseSkills & QualificationsStrong clinical assessment and treatment planning skillsExcellent communication and documentation abilitiesStrong time management and organizational skillsComfortable using EMR/documentation systemsProficiency with home health therapy equipment and adaptive devicesWork Environment & Physical RequirementsTravel within assigned geographic territory in varying conditionsWork in patient homes and assisted living environmentsAbility to lift up to 35 lbs (with assistance/devices as needed)Physical activity including bending, lifting, transferring patients, and stair navigation
