Responsibilities + Provide comprehensive primary care, including preventive care, chronic disease management, and acute visits. + Work closely with physicians and care teams to deliver coordinated, high-quality care. + Maintain accurate and timely documentation in the electronic medical record (EMR). + Promote patient education, health maintenance, and wellness initiatives.
