Health Advocates Network is currently seeking a **Nurse Practitioner** to work at a facility in **Rochester, NY.** These are _registry_ positions with our company. **$100 Sign-On Bonus** **Pay Rate:** $80 / hour **Shift details:** + M-F 8:AM-4:30PM Job description: Must have a New York State License to practice as a Registered Professional Nurse; and certification by the New York State Education Department as a Nurse Practitioner - Specialty in Adult Health or Family Health. Typical tasks include, but are not limited to: 1. Developing a health profile by taking health and developmental history, analyzing clinical laboratory data and other diagnostic tests, pertinent psychosocial data, and health assessment. Reviewing presenting problem, date of onset and duration. 2. Performing complete or focused physical examination to include blood pressure, weight and height. 3. Ordering routine laboratory tests such as CBC, blood chemistry, urinalysis, serology, as indicated. 4. Recording findings of health exam on appropriate forms. 5. Assessing data, discriminating between normal and abnormal findings. Identifying abnormalities that require further study. 6. Assisting in the development of the medical section of the treatment care plan as a member of the interdisciplinary treatment team. Evaluating the treatment plan for effectiveness and instituting necessary adjustments or corrections. 7. Making appropriate referrals to other community health care providers, specialists for consultation, tests, treatments. 8. Collaborating with other health disciplines to ensure continuity of care. 9.
