Nurse Practitioner
Job Description Conduct routine and comprehensive ESKD rounds under the guidance of nephrologists. Manage holistic patient care, including: Depression and mental health support Hospitalization risk reduction Transition of care and co-morbid condition oversight Provide patient education on ESKD and kidney disease. Perform annual disease management assessments. Collaborate with nephrologists, dialysis center teams, and other healthcare providers. Coordinate care with PCPs and specialists as clinically indicated. Support discharge planning and continuity of care.
