You’ll report directly to the Executive Director or Administrator and be assigned to a location to perform Face-to-Face (F2F) visits. In this role, you will assess patients and provide detailed clinical documentation to support the physician’s certification or recertification of hospice eligibility—based on CMS regulatory standards.Your assessments help ensure that patients continue to receive the compassionate care they need at the end of life.
