Nurse Practitioner – Post Transplant Care (Hybrid-Remote)

We are seeking an experienced and compassionate Nurse Practitioner (NP) to join our team providing holistic care to kidney patients—specifically kidney transplant recipients in Atlanta, GA.

This full-time hybrid-remote role includes both in-person and virtual responsibilities, combining the best of advanced practice nursing and nephrology care coordination. 

You will deliver high-quality care in a collaborative environment, performing Evaluation & Management (E&M) visits, reviewing labs, managing medications and referrals, and supporting complex patients through the healthcare system. Your efforts will directly support transplant recipients in their journey to support their new kidney and live a healthy life. 

Key Responsibilities: 

Perform in-person Evaluation & Management (E&M) appointments for transplant recipients 

Review labs and clinical data to make timely care plan adjustments in collaboration with supervising nephrologists 

Manage and adjust immunosuppressant therapies and chronic disease medications 

Handle prescription management, refills, and patient referrals 

Maintain a thorough and comprehensive EHR for each patient, including accurately recording the appropriate diagnosis codes and treatment codes 

Schedule & Work Environment:

Full-Time, Monday–Friday. Hybrid Remote. Must be flexible.

No nights, weekends, or on-call

Must be flexible to meet patient and nephrology partner needs

Travel within an assigned geography to dialysis clinics and nephrology practices

Qualifications:

Required:

Current unrestricted NP license to practice in the state. 

Minimum 2 years of NP experience in Family Practice, Internal Medicine, or General Medicine 

DEA license (or eligible within 90 days of hire) 

Current BLS/CPR certification 

Proficient in Epic or similar EHR system 

Intermediate computer skills with proficiency in Microsoft Word, Excel, and Outlook 

Home office setup with high-speed internet and reliable transportation 

Preferred:

Experience in nephrology, renal care, or transplant management 

Background in managing chronic diseases (e.g., CKD, diabetes, CHF, hypertension) 

American Nephrology Nurses Association (ANNA) Certification strongly preferred 

Spanish-speaking ability (preferred in certain markets)

Salary/ Wage Range

$110,100.00 - $161,700.00 / year
