**Cambridge Health Alliance (CHA)** , a well-respected, nationally recognized and award-winning public healthcare system is seeking a full time Clinical Specialist II, Nurse Practitioner, to join our CHA PACE Program. CHA PACE is a Program of All Inclusive Care for the Elderly (PACE) at Cambridge Health Alliance (CHA). CHA PACE serves adults 55 years and older with chronic medical and/or mental health difficulties with the goals of achieving the highest level of community independence and preventing nursing home placement. CHA PACE’s high functioning interdisciplinary team works to fully integrate and coordinate medical and behavioral health care, community resources, and in-home support services.
We are seeking a dedicated and experienced Nurse Practitioner to join our highly functioning interdisciplinary team. In this role, you will provide comprehensive psychiatric assessments, diagnoses, and treatment plans for elderly patients, focusing on their mental health needs in participants’ home, assisted living and nursing home settings. Essential job functions are focused on customer satisfaction, effective use of resources, teamwork, innovation and continuous quality improvement. **About the position:**
+ 36 Hours/week, M-F, MNA Cambridge Union (32/40)
+ The NP’s days will be assigned to either provide care at short term rehab (STR) facilities or GoTo coverage
+ NP works on site daily, either at our PACE center in Cambridge or at a STR center as assigned
+ GoTo coverage at the PACE center could include answering calls from home care nurses, nursing homes, triage nurses, clinic treatment nurses and pharmacy as well as occasional walk-ins and same day sick visits.
+ During STR coverage, the NP will follow patents and provide care in the facility

**Qualifications:**
+ Must be a graduate of an accredited Nurse Practitioner program, with a Master’s degree in Nursing
+ Current NP certification and licensure in Massachusetts.
+ MA. RN Licensure required
+ Current Massachusetts licensure is required authorizing practice in an expanded role as well as current DEA number and Mass DPH registration to prescribe controlled substances.
+ Minimum of 2 years of experience in geriatric health as an NP preferred.
+ Excellent communication and interpersonal skills.
+ Ability to work independently and as part of a collaborative team.
