**Schedule:** Monday thru Friday 8a-5p
**Position Summary**
We are seeking a compassionate and experienced **RN Nurse Navigator – Oncology** to join our multidisciplinary team. This role is essential in guiding patients through the complexities of cancer care, ensuring a seamless, coordinated, and patient-centered experience. The ideal candidate will bring a strong clinical background, excellent communication skills, and a passion for advocacy and education.
**Key Responsibilities**
+ Conduct comprehensive assessments of patients’ physical, psychological, social, and functional status.
+ Coordinate timely appointments and facilitate transitions across care settings.
+ Collaborate with physicians and multidisciplinary teams to develop evidence-based treatment plans.
+ Monitor and adjust care plans based on patient progress and evolving needs.
+ Identify high-risk patients and proactively address barriers to care.
+ Provide education to patients and families on disease processes, treatment expectations, and available resources.
+ Support patients through complex decision-making and end-of-life care planning.
+ Maintain oversight of disease-specific data registries and contribute to quality improvement initiatives.
+ Serve as a clinical expert and resource for staff education and internal consultation.
+ Participate in policy development, program planning, and performance improvement activities.
**Qualifications**
+ **Education:** Bachelor’s degree in Nursing preferred.
+ **Licensure/Certification:**
+ Current State of Michigan Registered Nursing License required.
+ Certification in subspecialty (e.g., OCN, CNRN) required.
+ Additional subspecialty certifications preferred.
+ **Experience:**
+ Minimum 3 years in an acute care setting.
+ At least 1 year in medical, surgical, or radiation oncology required.
+ Case management or patient care leadership experience preferred.
+ 5 years in subspecialty preferred.
**Additional Requirements**
+ Must be able to work flexible schedules to meet patient and program needs.
+ Strong critical thinking, problem-solving, and communication skills.
+ Ability to work collaboratively in a fast-paced, multidisciplinary environment.
