urgent care and more than different specialtiesJob DescriptionResponsible for coordinating the individualized care of patients throughout the continuum of care services from abnormal findings of a lung nodule to end of treatment in order to increase and assure timely access to healthcare services Will be responsible for helping manage patients who have incidental lung nodules as well as nodules found from lung cancer screening exams Will be an instrumental part in helping grow the program for the systemServes as a member of the multidisciplinary care team between thoracic surgery pulmonary and radiology functioning as a patient advocate educator key point of contact and liaison between the patient and all other appropriate servicesManages referrals schedules consultations order and schedule appropriate testing in a timely and efficient manner Follows up on diagnostic testingNavigates patients through the continuum to eliminate barriers to health care and enhancing patient and physician satisfactionFunctions as a collaborating member of the health care team but also must be able to act independently to assess patient and provider needs to ensure appropriate handoffs so that patients receive services promptly and efficientlyApplies the nursing process as appropriate within the context of the organizationrsquos framework for professional practice and in accordance with guidelines established by the teamParticipate as needed in community education outreach programs workshopsseminarssupport servicesProvide coverage as necessary for thoracic surgery nursing responsibilities that may include triaging patient calls pre operative instructions and consultation managementUtilizes EPIC for tracking data to help improve the program and increase efficiency in following patientsResponsible for weekly MDC meeting which includes moderating meeting managing agenda managing CME taking minutes and attendance and communication with multidisciplinary teamHartford
