Center CFMDC a signature program of the Ayer Neuroscience Institute This role provides guidance for patients through the continuum of care to eliminate barriers and enhance patient outcomes and satisfaction The Nurse Navigator provides education support and logistical assistance helping patients navigate the complexities of diagnosis treatment and recovery This includes communicating the plan of care with patients families and providers across specialties to assure coordination throughout the continuum of care Additionally the Nurse Navigator will serve as a professional resource for data tracking and process improvement Key priorities for this role include supporting timely access to patient care facilitating effective communication and navigation across the care continuum and enhancing the overall patientfamily experience and quality of care The ideal candidate is an experienced registered nurse with a background in neurology or movement disorders or a strong interest in
