of our patients The Nurse Navigatorserves as a patient advocate liaison and advisoreducator to guide patients throughout the continuum of care for their surgical experience beginning in the pre operative phase up to a full year post operatively to mitigate risks and promote positive outcomes This position builds relationships with patients families and the internal clinical team to improve patient outcomes satisfaction and transitions through the continuum of careJob Responsibilitiesmiddot Serves as a member of the multidisciplinary care team functioning as a patient advocate educator key point of contact and liaison between the patient and all other appropriate servicesmiddot Navigates patients through the continuum of care working with the multidisciplinary team to enhance patient and colleague experiencemiddot Utilize electronic medical record reports and data management skills to coordinate patient flowmiddot Participate in community and colleague education Certification in specialty area preferredmiddot Minimum years clinical nursing experience preferred particularly with orthopaedic or neurosurgical patientsmiddot Competent with standard software Word Excel Electronic Medical Record and willing and able to learn additional software as needed to enhance navigation and coordination responsibilitiesmiddot Knowledgeable regarding data interpretation and the tracking and evaluation of quality indicatorsmiddot Demonstrates use of clinical skills and expertise in diagnosis treatment and psychosocial impact of diseasemiddot Demonstrates excellent customer service skills including superior telephonic communications skills with the ability to comprehensively assess patientrsquos physical and psychosocial status as well as conduct complex medical history discussions over the telephonemiddot Ability to effectively and efficiently coordinate implement and monitor the patientrsquos process of care to communicate openly and empathetically with patients and their family membersmiddot Ability to work independently as well as collaboratively with navigation team and multidisciplinary care team in all aspects of defining and implementing navigation programmiddot Ability to communicate effectively with diverse audiences including professional staff patients community media and other service agenciesmiddot Knowledge of adult learning competencies and patient teaching skillsmiddot Demonstrated management skills reflecting self motivation initiative and fiscal responsibilitymiddot Tolerance of ambiguity and the flexibility to
