Nurse Care Navigator Responsibilities: 

Work with medical staff to create patient care plans  Work one-on-one with patients and newborn to ensure their needs are met  Develop and implement policies and protocols that will ensure the success of our program  Serve as point of contact between providers and patients  Be a familiar, reliable, and accessible resource for patients as they navigate a stressful time  Obtain patient outcomes after delivery   Ensure that all HIPAA regulations are followed
