continuum eliminating barriers to healthcare and enhancing patient and physician satisfaction Serves as a member of the multidisciplinary Heart team functioning as a patient advocate educator key point of contact and liaison between the patient and all other appropriate services after discharge Coordinates and works collaboratively to ensure efficient transition of care for transferred Structural Heart patients middot Provides support to patients and caregivers through guidance direction and liaison in both inpatient and outpatient settingsmiddot Participates with Structural Heart practitioners and physicians in the outpatient clinic to complete medication reconciliation education check in and rooming of patients when necessary and assists in coordinating testingmiddot
