Position Summary: The Oncology Nurse Navigator is a patient educator and advocate, and care coordinator that helps patients diagnosed with cancer navigate through the Guthrie health system to improve patient experience and quality of care. The Oncology Nurse Navigator acts as a resource for the patient and family through diagnosis and treatment of cancer. Experience: + A minimum of 4 years’ experience in nursing, possess a functional knowledge of cancer diagnosis and treatment, and demonstrate the ability to work collaboratively with a multidisciplinary team to educate and coordinate care for patients. + Oncology experience preferred. Education: + Graduate of an Accreditation Commission for Education in Nursing (ACEN) or Commission on Collegiate Nursing Education (CCNE) accredited nursing program. License & Cert + Licensed Registered Nurse in the state of position. + Specialty certification is recommended as soon as the RN is eligible. Essential Functions: + Supports patient from diagnosis throughout treatment during entire cancer care continuum. + Coordinates patient care with other departments as well as within the Cancer Center. + Works in collaboration with other disciplines to provide quality care. + Triages calls to the hematology/oncology office from patients + Works with oncology providers to address patient concerns over the phone and in the office + Uses nursing judgement and critical thinking to direct patient care. + Participates in community outreach programs and activities + Promotes educational programs internally and externally in areas such as prevention, screening guidelines, diagnosis, treatment options, early detection and identifying of at‐risk populations. + Participates in multidisciplinary conferences with physicians and clinicians to discuss best treatment plan for patients. + Assists patients in obtaining the required referrals, outside studies or second opinions + Acts as a liaison between patients and other health care providers + Conducts follow‐up calls or contact with patient throughout course of treatment to continually assess patient needs.
