Major Responsibilities:
Participates in the direct care of patients.
Assess, plan, implement and evaluate care of women and men with breast related health concerns. Teaches patients about the importance of screenings such as mammograms, colonoscopy, etc.Assess patient’s emotional needs and/or distress using the standard distress thermometer on a regular basis. Provides emotional support.Assists patients and staff in the appointment process to expedite timely appointments.Provides patients with appropriate education related to prevention, diagnosis, tests, surgery, chemotherapy and radiation therapy, etc, which includes a Survivorship Care Plan (SCP).Work with physical therapy and rehab to develop lymphedema screening assessment program which includes tracking process and monitoring compliance with treatment.Partners with survivorship coordinator to transition breast cancer survivors to a practice dedicated to supporting these patients and their appropriate follow up (imaging, medications, support, resources, etc).Coordinates testing and expedites results reporting.Provides patient's family with resources for fiscal management.Assures continuity of care by interfacing with day surgery, in-patient units and out-patient facilities.Works with patients for post mastectomy garments.
Participates in indirect care related to patients.
Follow-up with physicians on all abnormal testing in a timely fashion. Follows up with patients in a timely and professional manner.Develops educational tools for patients. Documents all education, assessments and care given according to hospital standards.Identifies, collects, monitors and evaluates quality issues related to clinical practice in conjunction with interdisciplinary team to improve patient outcomes and resource allocation.Establishes method for tracking patients progress through the entire episode or continuum of care.Acts as a resource in promoting cancer awareness, education and screening tools.Acts as a resource for patients and other employees.Uses effective listening and questioning techniques in a professional and empathetic manner, demonstrating sensitivity to patients and families.Works with local and regional support groups in relation to primary referral diagnosis.Active participant in case review, tumor board, and Breast Program Leadership Committee.Coordinates multidisciplinary approach for the patient.
Participates in Community Education
Identifies, develops, and participates in outpatient and community programs related to cancer screening, diagnosis, and cancer related health issues.Develops education programs for cancer screening programs and awareness.Participates in Cancer Committee activities.Works with all departments to provide seamless care for patients recently diagnosed with breast cancer.Teaches other departments and employees about breast cancer education, screening tools and treatment options.Develops educational programs for the public and preferred community partners.
Professional activities.
Responsible for continued self-growth and education in areas related to changes in cancer detection, education, and treatment options.Presents a professional image and courteous manner representing Advocate Aurora Healthcare to the public.Participates in department and clinic activities to promote service excellence.Promotes team behaviors and acts as a role model for others.In partnership with department leadership and breast program leader, champion the NAPBC accreditation process, clinically and administratively.Maintains accurate, legible, and confidential recordsResponsible for fundraiser activities/opportunitiesOther duties as identified
Licensure:
Nurse, Registered (RN)
Education/Experience Required:
Bachelors Degree Nursing or related field.3-5 years focused experience in clinical areas.CBCN Certification within two years of hire.Utilization of critical thinking and timely decision making Documented training in patient navigation.
Knowledge, Skills & Abilities Required:
Effective interpersonal communication skills Global perspective of changing healthcare environment.Ability to prioritize work.Ability to access, plan, intervene, and evaluate the physical, emotional and educational needs of patients.Knowledge of computer operations and applications. Spanish speaking preferred.Current RN license in the state of Illinois. CPR certification.
