We partner with our Nurses at Wesley Medical Center! **Job Summary and Qualifications** **The Nurse Navigator is responsible for the programs and resources that support the navigation of patients through complex healthcare environments for the optimization of care and outcomes. Facilitates patient follow-up through coaching, information transfer, and communication in order to improve continuity of care, enhance the medical management of complex disease, and streamline the intervention process.** **Duties as a Nurse Navigator will include** **:** ** ** + **You will guide patients through the health care system by assisting with access issues, developing relationships with service providers, and tracking interventions and outcomes.** + **Your role serves as a point of contact for referring physicians, patients and caregivers to provide resources and assistance with accessing clinical and supportive care services.** + **You will review patient case lists generated by technology-enabled data triggers to identify patients that could benefit from intervention. ** + **Your role maintains detailed records to measure effectiveness of program and suggest ways to improve throughout the process, as well as tracks outcomes of referrals and treatments. ** + **You will review medications, educate patients as needed, and initiate communication regarding dashboard triggers with care team. ** **Qualifications for a Nurse Navigator:** + **Current licensure in the State of Kansas as a Registered Nurse (RN) or Current Multi-state licensure as a Registered Nurse** + **Current Basic Life Support (BLS) course through the American Heart Assoc. or American Red Cross – Healthcare Provider CPR** + **Graduate from an accredited school of nursing – BSN preferred** + **Experience as a nurse navigator and/or patient educator preferred** + **RN experience in the ICU and/or Med/Surg preferred** **Benefits** Wesley Medical Center, offers a total rewards package that supports the health, life, career and retirement of our colleagues.
