What You’ll Do: + Lead & Mentor: Supervise field nurses, provide ongoing education, and conduct performance evaluations. + Manage Patient Care: Oversee admissions, care planning, and case management with a focus on quality and compliance. + Ensure Regulatory Excellence: Conduct chart reviews, home visits, and ensure adherence to CMS CoPs and state regulations. + Collaborate & Communicate: Work closely with physicians, caregivers, and families to develop individualized care plans. + Drive Quality: Participate in QAPI initiatives, audits, and policy development. + Support Technology: Utilize EMR systems (Homecare HomeConnect) and Microsoft Office for documentation and reporting. + Be Available: Participate in on-call rotation to provide clinical support when needed. What You Bring: + Active RN license in Connecticut; BSN preferred. + 5–7 years of nursing experience, with 3–5 in home health. + 1–2 years of supervisory or management experience. + Expertise in OASIS, care planning, and CMS CoPs. + Strong leadership, mentoring, and team-building skills. + Proficiency in EMR systems (Homecare HomeConnect) and Microsoft Office. + Valid driver’s license and ability to travel locally.
