About the Role
Are you a passionate nursing leader ready to make a meaningful impact in home health care? We’re seeking adynamic and experienced Nurse Managerto lead our clinical teams with empathy, precision, and purpose. In this pivotal role, you’ll oversee patient care delivery, ensure regulatory compliance, and mentor a team of dedicated field nurses—all while championing quality, innovation, and compassionate service.
What You’ll Do
+ Lead & Inspire: Supervise, coach, and evaluate field nurses to ensure exceptional patient outcomes and professional growth.
+ Oversee Clinical Operations: Manage admissions, care planning, and case management; monitor patient progress and adjust care plans as needed.
+ Ensure Compliance: Conduct chart reviews and home visits to uphold CMS guidelines, state regulations, and internal policies.
+ Drive Documentation Excellence: Ensure timely, accurate completion of OASIS, Plans of Care, and 485s.
+ Collaborate Across Care Teams: Partner with physicians, caregivers, and families to create personalized, holistic care plans.
+ ⚕️Support Talent Development: Assist with hiring, onboarding, and continuous clinical training.
+ Champion Quality Initiatives: Participate in QAPI programs, audits, surveys, and policy development.
+ Leverage Technology: Utilize EMR (Homecare HomeConnect) and Microsoft Office for efficient documentation and reporting.
+ Be a Clinical Resource: Provide support during on-call shifts as needed.
What You Bring
+ Licensure & Education: Active RN license (BSN preferred).
+ Experience: 5–7 years of nursing experience, including 3–5 years in home health and 1–2 years in a leadership role.
+ Expertise: Deep knowledge of OASIS, CMS guidelines, and care planning.
+ Tech Savvy: Proficient in EMR systems (Homecare HomeConnect) and Microsoft Office Suite.
+ Soft Skills: Strong communicator, critical thinker, and problem-solver with a passion for mentoring and team development.
+ Mobility: Valid driver’s license and ability to travel locally.

Pay and Benefits
The pay range for this position is $140000.00 - $150000.00/yr.
Paid Time Off & Holidays15 PTO days annually after 90 days, usable for vacation or sick time.Accrual: +1 day per year of service, up to 20 days max.8 paid holidays: New Year’s Day, MLK Day, Memorial Day, Juneteenth, July 4th, Labor Day, Thanksgiving, Christmas.Flexibility: Unused PTO beyond 15 days can be paid out upon request.
