Nurse Manager, Case Management

Description:

The Nurse Manager of Case Management is responsible for the 24/7 daily operations of the MacNeal Hospital Case Management department responsible for the coordination of timely inpatient care delivery and safe transition planning. The Nurse Manager works with the multidisciplinary team including physicians, colleagues and payers to ensure patients’ progress along the continuum of care in an efficient and cost-effective manner that ensures quality outcomes.

Duties include colleague position control and personnel management; staffing/scheduling, establishing and maintaining policy and procedure, budget planning and productivity management, review of department performance and programmatic impact. They oversee performance improvement projects to achieve department targeted outcomes. They facilitate multidisciplinary rounds and support staff in development of individualized, evidence-based, outcomes oriented, and safe practice. The Nurse Manager of Case Management develops and monitors processes to ensure alignment with Trinity Health values and culture. The role is influential in creating a professional environment where interdisciplinary team members contribute to optimal patient outcomes. They are responsible for creating a safe, healthy work environment that supports the work of the care team, encourages professional development and contributes to patient engagement.

The Nurse Manager of Case Management is responsible for influencing the strategic plan and goal planning in conjunction with the Director of Case Management, serving the Loyola Medicine hospitals and Trinity Health Care Management. This position will report to the Regional Director of Case Management. They serve as an active member of the hospital and local health ministry leadership team with the principal goal of providing care management services for patients, visitors, and colleagues.  They serve as the key contact within the hospital to align Case Management vendor processes. They ensure ongoing process improvement within the Case Management scope and partners with Population Health, Clinically Integrated Network and Accountable Case on all longitudinal plan of care initiatives. They ensure compliance with all local, state and federal regulatory requirements.

What's required:

Master's degree in nursing or health-related field.3-5 previous job-related experience.Prefer 3-5 years of management experience.Current RN license in the State of Ilinois.Certification in Case Management preferred.

Compensation:

Pay range: $96,782- $159,702

Actual compensation will fall within the range but may vary based on factors such as experience, qualifications, education, location, licensure, certification requirements, and comparisons to colleagues in similar roles.
