What You'll Do:
Holistic Assessment & Planning: Develop personalized, comprehensive plans of care through patient/family interviews, physical assessments, and psychological history.Expert Case Management: Document skilled care, treatments, and diagnostic procedures, ensuring meticulous records.Continuous Evaluation: Assess patient status on admission and subsequent visits, regularly inform physicians of changes, and coordinate interdisciplinary services.Compliance & Documentation: Complete initial evaluations, recertification, discharge summaries, and all federally required forms (e.g., Medicare).Shift: 8am - 5pm
Core Schedule: Monday - Friday; Includes On Call Rotation
Job Requirements
What You'll Bring:
