**Job Description**
**Job Title:** **Nurse Clinical Coordinator – Care Management**
+ Mount Sinai Health Partners (MSHP) is the management services organization developed to enhance the Health System’s implementation of population health initiatives, that is, programs of care that enhance the patient experience and health status, improve the health of patient populations and reduce utilization and healthcare costs.
+ Population health initiatives particularly focus on preventive care. MSHP provides the infrastructure to support population health initiatives and includes services such as care coordination, information technology, workflow optimization, physician engagement and quality reporting.
+ The Department of Social Work Services takes a leadership role in MSHP Care Management, which is providing in person and telephonic short-term and long-term care management services for a large diverse group of individuals who have a variety of insurances that are part of value based contracts.
+ The goal of MSHP Care Management is to provide standardized care management services to meet patients’ medical and psychosocial needs, increase quality and reduce unnecessary health care costs.
**Qualifications**
+ Bachelor’s degree in specific discipline (i.e., Bachelor of Science in Nursing).
+ Master’s degree in health related field is preferred.
+ Certification in specialty field is preferred
+ 2 years’ experience in specialty clinical field.
+ Current NYS nurse licensure.
+ Computer skills - MS Office Suite (Intermediate level).
+ Communication and interpersonal skills to work effectively with a wide variety of individuals.
+ Ability to use basic data entry and retrieval systems.
+ Ability to organize information and teach others.
+ Analytic ability to collect and organize information identifies problems and trends, formulate and implement corrective plans or programs.
+ Ability to coordinate, prioritized, and allocates resources.
+ Oriented to consumer satisfaction.
+ Able to coordinate a broad variety of services and programs for designated patient population on a daily basis.

**Compensation Statement**
The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $68.49 - $68.49 Hourly. Actual salaries depend on a variety of factors, including experience, education, and operational need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.

**Responsibilities**
+ The Nurse Clinical Coordinator (NCC) is responsible for the complex, multi-disciplinary coordination of specialty patient population(s), developing, evaluating and revising care pathways based on assessments conducted through engagement with the patient, family and multiple departments and disciplines.
+ The NCC interfaces directly with MSH Departments, Administrators, Nursing, Medical and Quality leadership, and clinical staff and coordinates care for a patient population regarding the clinical, educational and operational impact of all care delivery, and outcome measures.
+ The NCC reports directly to discipline leadership with a professional line to MSH Nursing or related disciplines professional practice leadership. The NCC is responsible for developing and implementing a plan of care for a specialty patient population, and/or specialty specific standards of care organization wide.
**_This is a NYSNA position with PTO. The position currently follows a hybrid work model (_** **_combination of remote work and office-based work at 150 East 42_** **_nd_** **_street, Manhattan)._**
