Position Summary:  Under direct supervision, provides personal and homemaking services to ill, convalescing, elderly, and disabled persons in the patients' home. Essential Functions and Responsibilities:1. Provides personal care of a patient, including bathing, grooming/hygiene, and toileting.2. Takes and records daily vital signs. Reports all vital signs that are outside the normal range.3. Assists patients with positioning/turning, transfers, catheter care, and range of motion exercises.  #LI-CC1
