**Responsibilities** ESSENTIAL KEY JOB RESPONSIBILITIES 1. Communicates with clinical, clerical, providers, patients, and families to ensure continuity of care. 2. Review and/or initiate a phone note or task in electronic record system. (May not perform telephone triage.) 3. Ensures that the medical record and documents are accurately complete. 4. Escorts patients to exam rooms, obtains accurate vital signs and obtains medical data such as patients history, symptoms, and reason for visit. (May not complete screenings, assessments or medication review or reconciliation.) 5. Prepares and maintains exam rooms and pod area with supplies and equipment for treatment 6. Employees may be required to perform other duties as assigned **Qualifications** **Minimum Qualifications:** + (1) year experience as a Medical Assistant in an outpatient medical office, physician clinic or OB Office setting.
