Standard Job Description:
+ Plans, directs, coordinates, and implements spine-specific care programs. Evaluates patient status, educates staff on specialized techniques, and serves as a clinical resource to the multidisciplinary health care team.
Program-Specific Responsibilities:
+ Serves as a key member of the Spine Pathway Team, supporting its development, implementation, and ongoing evaluation.
+ Collaborates with Emergency Departments (EDs), Primary Care Providers (PCPs), and other referral sources to ensure patients with appropriate ICD-10 diagnoses receive timely follow-up and engagement, including imaging, pain management, and surgical consultations.
+ Partners closely with Jenna, the Spine Institute team, Lenox Hill Hospital (LHH) clinical teams, and social work to coordinate care and streamline transitions.
+ For surgical patients, works in collaboration with physicians, Advanced Care Practitioners (ACPs), residents, and hospitalists to ensure a seamless patient experience from admission through discharge.
+ Anticipates discharge needs and facilitates optimal disposition planning.
+ Monitors and tracks pathway compliance, length of stay (LOS), Press Ganey satisfaction scores, and post-discharge follow-up coordination.
General Responsibilities:
+ Reviews clinical protocols to maintain quality control and patient safety, and supports physicians in promoting patient compliance.
+ Participates in quality and case review meetings to assess clinical outcomes and identify areas for improvement.
+ Provides education to patients, families, and health care professionals regarding spine care and recovery.
+ Performs related duties as required. All responsibilities noted are considered essential functions under the Americans with Disabilities Act. Duties not listed but related are considered non-essential functions.
Qualifications:
+ Bachelor's Degree in Nursing (BSN) required.
+ Current license to practice as a Registered Professional Nurse in New York State required.
+ 3-5 years of clinical experience required; 0-2 years of leadership or management experience preferred.
+ Spine care experience and relevant certifications strongly preferred.

When determining a team member's base salary and/or rate, several factors may be considered as applicable (e.g., location, specialty, service line, years of relevant experience, education, credentials, negotiated contracts, budget and internal equity).
The salary range for this position is $78,000-$130,000/year
