**What you'll do:**
A hepatology social worker primarily focuses on providing psychosocial support and resource navigation to patients with liver disease, including those undergoing liver transplants, by assessing their social, emotional, and financial needs, connecting them with appropriate community services, and assisting them in managing the complex challenges associated with their condition and treatment plan.
**Key responsibilities of a hepatology social worker include** :
**Psychosocial assessments** :
Conducting comprehensive evaluations of patients' emotional, family, and financial situations to identify potential barriers to treatment and understand their coping mechanisms.
**Individual and group counseling** :
Providing therapy sessions to patients and their families to address concerns related to diagnosis, treatment side effects, lifestyle changes, and end-of-life planning.
**Care coordination** :
Collaborating with the hepatology team, including doctors, nurses, and dietitians, to ensure patients receive coordinated care and access necessary resources.
**Resource linkage** :
Identifying and connecting patients with community support services such as financial assistance programs, housing support, transportation assistance, and support groups.
**Advocacy** :
Representing patients' needs to insurance companies and other healthcare providers to navigate complex medical billing and access to treatment.
**Education** :
Providing patients and families with information about liver disease, treatment options, and coping strategies.
**Crisis intervention** :
Responding to urgent situations, such as emotional distress or sudden changes in health status, and providing immediate support.
**Post Discharge planning** :
Assisting with post-discharge planning, including coordinating follow-up appointments, home healthcare needs, and community support services.
**What you’ll need for this job:**
+ Minimum Education: Required: Master’s Degree in Social Work
+ Minimum Experience: Preferred: 1-2 years of previous job-related experience
+ Licensure/Certifications: Required: Licensed Social Worker State of Illinois Preferred: Licensed Clinical Social Worker State of Illinois in Hospital Setting Preferred
+ _Will consider a license pending MSW. Candidate must achieve LSW with 6 months of hire. Candidate will also receive weekly individual supervision._
+ Graduate level internship in hospital setting with minimum of 1-year intensive training required.
**Compensation:**
Salary Range: $30.00 - $41.59 per hour
_Actual compensation will fall within the range_ _but_ _may vary based on factors such as experience, qualifications, education, location, licensure, certification requirements, and comparisons to colleagues in similar roles._
