Job Summary:\rThe nurse manager of the home care program is a registered professional nurse who provides leadership and management of the home care unit through the effective use of home health knowledge and home care nursing skills to create a center of excellence that promotes the mission and vision of WellStar Home Health. The nurse manager is responsible for the delivery of interdisciplinary care by the professional and non-professional staff in the home setting. The Manager oversees the episode of care, coordinating team members, and maintaining quality of care as well as acting as a communication resource between the agency, staff, physicians and other entities. The Manager is responsible for effective utilization of services in a fiscally responsible manner The nurse manager is the standard bearer of his/her unit and supports the RN Case Managers to hold the LPNs accountable/ responsible for the quality outcomes, allocation of resources (both human and material) and adherence to all state, federal and accreditation requirements. He/She fosters an environment of trust by open communication, promoting teamwork, inclusion and respect. The nurse manager Role specific competencies, responsibilities, roles and functions are based on the Professional Practice Model, 5-Star Nursing. It is expected that all RN Clinical Nurses are licensed, knowledgeable and uphold the practice of nursing as outlined by the Georgia Professional Nurse Practice Act and implements the Scope of Practice and Code of Ethics Standards put forth by the American Nurses Association. As a member of the patient services team, it is expected the individual upholds the voice of the patient, system policies and procedures while supporting service excellence goals.\rCore Responsibilities and Essential Functions:\rPeople\r* Supervise direct reports and ensure productivity management expectations are met\r* Interview and evaluate applicants for open positions within area of responsibility\r* Responsible for employee engagement, department retention, and turnover\r* Perform on-site visits with field clinicians , ensure training and development of staff is complete\r* Complete mid-year and yearly evaluations on direct reports\r* Work with HR and Director provide corrective actions and resolve complex issues timely\r* Manage Employee HR information through Success Factors\rQuality and Patient Safety\r* Maintains clinical knowledge and skills by keeping current of trends, procedures and techniques\r* Review daily active patients to ensure all patients were scheduled and notes complete\r* Work in conjunction with the QA team to ensure accurate and timely SOCs and recerts\r* Conference with project assistant daily to ensure productivity is being met, visits and SOCs are scheduled, new patients have visits\r* Accountable for compliance with regulatory and/or accreditation standards as appropriate\r* Develop and conduct weekly team conferences to provide clinical education and patient review\r* Review all written POCs, review and process all PIOs & LPN orders, obtain orders from MDs\r* Ensure patient satisfaction and clinical outcomes through HHCAHPS reports\r* Serve on Committees as assigned\r* Review all new admissions to ensure appropriate referrals with needed supplies and orders\rCustomer Service\r* Serves as a communication resource between Wellstar Home Health, patients, physicians, and other entities\r* Ensures effective communication with other Wellstar departments and team members\r* Serves as resource to perform any service recovery as necessary\rFinancial\r* Works in conjunction with the scheduling team to ensure continuity of care and established productivity expectations are being met\r* Monitors the volume of patients daily to determine productivity management and the need for additional resources\r* Ensure Discharges are completed timely\r* Works with the Home Health Director is ensure cost control initiatives are being met\r* Review Drop ship supplies to ensure cost savings and appropriateness\rSuccessful Growth\r* Works in conjunction with Home Health Director and Executive Director regarding strategic planning and process improvement\r* Support the Home Health targeted growth budget\r* Recommends and implements any new programs or services for department\rPerforms other duties as assigned\rComplies with all WellStar Health System policies, standards of work, and code of conduct.\rRequired Minimum Education:\rGraduate of an accredited school of nursing\rBachelor's Degree\r\rRequired Minimum License(s) and Certification(s):\rAll certifications are required upon hire unless otherwise stated.BLS - Basic Life Support or BLS-I - Basic Life Support - InstructorRN - Reg Nurse (Single State) or RN-COMPACT - RN - Multi-state CompactGADL - Georgia Driver's License or VSDL - Valid State Driver's LicensePVI - Private Vehicle insurance\rAdditional License(s) and Certification(s):\rRequired Minimum Experience:\rMinimum 1 year in the Medical Required or\rsurgical (Nursing), Clinical setting Required and\rMinimum 1 year of successful supervisory/management and Homecare Experience Required\rRequired Minimum Skills:\rShould possess excellent verbal and written communication skills and relationship building; a knowledge of the health care environment; leadership that includes such skills as successfully implementing and managing a change process ability to use systems thinking- and inspire, coach others to be successful; model the way for professional practice; professionalism and teamwork/collaboration for self/department/medical staff; and possess necessary business skills to manage human and material resources.\rMust be a continuous learner who understands health care financing; strategy and operations for running a business unit(s); and effectively employ data and technology to support work processes and make decisions.\rSupports shared governance or shared decision making; manage the design and delivery of care that in based on evidence and focused on quality and safety.\rKnowledge of risk management and various accreditation/regulations/licensing requirements assuring that these are upheld while advancing industry standards through the achievement of goals by effective management and measurement of outcomes.
