Short Description

The Mental Health Specialist is a newly required position on the PPC grant. This position supports the department through direct outreach to PCPs, patients, and community resource partners. This position is an integral part of the team, providing follow-up services to PPC stakeholders

 

Provides clinically competent case management services to clients as needed
Makes phone calls, communicates with patients and families, and completes disposition letters for the PCP’s.
Provide referrals and linkage to community-based services, and case management for patients and their families.
Participates in daily clinical team meetings and documents updates in client EMR records.
Utilizes My Cooper when communicating with patients.
Extracts information from EMR and utilize it to identify referral sources, time from referral to intake, and various other benchmarks.
Creates telephone encounters, messages and schedule appointments in EMR. 
Maintains work queues, proper cash handling and end of day procedures, scheduling patients according to template and protocols, answering and processing telephone calls, successful check in/out of patients and verifying all demographics. 
Fulfills responsibilities as assigned including sending/receiving patient medical records. Obtaining lab reports, hospital notes, and referral information. 
Completes forms/requisitions as needed and manages charts to ensure that information is complete and documented appropriately in health maintence.

Bachelor's Degree Preferred
